Breast augmentation is without doubt the most demanded plastic surgery procedure in Spain at the present time. Patient ages range from 18-20 to 50 years old with some exceptions at both ends.

This is a procedure that brings about a much greater psychological impact than it would seem at first glance. Why would a woman undergo a surgical procedure to gain a bigger chest?

This is a question that could be perfectly answered by many women, while many men wouldn’t be able to adequately do so. A large part of a woman’s sense of femininity resides in the physical attributes of the female body: curves, contoured hips, a defined waist, rounded backside and legs, and of course a prominent chest. It is not about simply being a woman, but feeling evenmorelike a woman.

These and other reasons are responsible for the Breast Augmentation procedure having become the fastest growing Plastic Surgery operation worldwide in the last few years, the one which we plastic surgeons perform most often, and the one which provides the most satisfaction to our female patients. There is a phrase that every professional in this field with some years of practice knows well: ‘I should have got it done earlier’.

MEDICAL CONSULTATION

During her first meeting with the surgeon the patient will receive a thorough explanation of the procedure. She will also undergo a physical exam to measure every parameter necessary to determine the level of breast augmentation that she desires, or the one most suitable to her chest anatomy and physical make-up. The patient will have to request a series of lab analyses and medical tests to guarantee that she won’t incur any risk to her health during the operation.

Variables such as skin type, skin elasticity, current mammary volume, level of sagginess if any, projection, musculature, etc. will be evaluated during the consultation.

Based on such data the surgeon determines the best type of prosthesis and the volume that better fits the needs and tastes of the patient. It is better to recommend an implant size that satisfies the patient’s expectations without exceeding what is suitable to her physical constitution and being harmoniously proportioned.

In order to accurately choose the prosthesis’ size and establish its most appropriate dimensions we measure out the desired length, height and projection.

ANAESTHESIA

During a Breast Augmentation procedure it is advisable to use general anaesthesia. The patient stays asleep during the entire operation which allows for a better relaxation of the pectoralis major muscle under which the prosthesis is inserted in most cases. This way the patient feels absolutely no pain at all. In the case of subglandular implantation the procedure can be done with local anaesthesia plus sedation.

SURGICAL TECHNIQUE

Breast Augmentation technique allows for three implantation positions:

Nowadays the most common placement of the implant is the submuscular position (under the chest muscle) as it allows for a better ‘cushioning’ of the prosthesis when implanted into the body of a slim patient.

The next type of placement is the subglandular one, recommended for women who wish for a moderate breast size increase and have enough chest volume to provide appropriate padding to the implant.

The third one is subfascial placement (under the outer surface of the chest muscle) which is more suitable for patients who have a good chest cushioning already and don’t want a big increase in volume (this type of positioning is an improvement on the subglandular type).

There are three possible approaches to the insertion of the prosthesis: periareolar (the most common type of incision), inframammary and transaxillary. Each one has its own advantages and drawbacks, the periareolar incision being the most advantageous and having the least drawbacks.

The intervention lasts approximately one hour.

HOSPITALISATION

The Breast Augmentation procedure can be done on an overnight hospital admission basis or as an outpatient: choosing one or another depends on factors specific to each patient such as medical advice, personal circumstances, scheduling of surgery, type of anaesthesia, distance from home, etc.

POSTOPERATIVE PERIOD

Postoperative pain in the days immediately after surgery varies depending on the patient’s condition, the size of the implant and its placement, sensitivity to pain, etc. In order to provide comfort to the patient the doctor will prescribe the use of analgesics.

Edema (swelling) in the breasts and a feeling of discomfort upon raising the arms are usual at the beginning of the postoperative period.

The first bandage dressing will be removed 48-72 hours after the procedure and will be replaced with an custom-fitted underwire-less elastic bra. It is recommended to wear this bra for approximately one month 24 hours a day (day and night).

Stitches, if non-absorbable, will be removed after 10-15 days after surgery.

It is advisable to anticipate a convalescence and off-work period of roughly 6 to 10 days.

Waiting one or two months before resuming any sport activity is also recommended.
